MEDICAL RECORDS
Nothing is more important in getting you qualified properly.

The most important item in the process of qualifying you for Medical Marijuana use is the MEDICAL RECORDS that support the diagnosis of the condition for which are you seeking medical marijuana.
Those supporting records are absolutely necessary for the physicians at SWFL Medical Marijuana to issue the qualifying opinion that is required for you to receive the Medical Marijuana recommendation you seek.

How to Get Medical Records

Our staff will work for and with you to get your medical records via our printable Medical Records Release.

Page 1: Now fill out the Medical Release form but please carefully follow instruction on top of form. We need all lines that have an (X) on it completed.

Page 2: This is where you tell us where to look for your records and the more info you can give us the faster we can get them. We are looking for the physicians, clinics and hospitals that would have records related to your specific condition, we need as much info on those people and places like names, location, phone and fax #’s the more you assist the better it will go. We are also looking for pathology like labs, MRI reports, CT reports, X ray reports so tell us the facilities that did those scans. REMEMBER WE PREFER REPORTS OVER FILMS

IF YOU ALREADY HAVE MEDICAL RECORDS (partial or complete) WE NEED THEM, SO PLEASE MAKE COPIES FOR OUR USE, IN YOUR CHART AND GET THEM TO US – WE WILL STILL NEED THE RECORDS RELEASE AND PATIENT REGISTRATION TO SET AN APPOINTMENT.
 
Once all your records are received and your medical chart is fully completed we will contact you to schedule your exam but please understand that getting records takes some time.
